Understanding Your Results
What the Wellness Score Measures
The Health Risk Assessment evaluates five evidence-based wellness domains that the CDC and the U.S. Preventive Services Task Force (USPSTF) have identified as key indicators of preventable disease risk. These domains include regular physical activity (strongly correlated with reduced cardiovascular disease, type 2 diabetes, and certain cancers), adequate fruit and vegetable consumption (associated with lower blood pressure, improved cholesterol profiles, and reduced inflammation), timely preventive health screenings (critical for early detection of hypertension, diabetes, and cancer), effective stress management and mental wellness (linked to immune function, cardiovascular health, and all-cause mortality), and tobacco and nicotine abstinence (the single most impactful modifiable risk factor for premature death).
Score Interpretation
Scores ranging from 12 to 15 (80% or higher) indicate a strong wellness profile with health behaviors well-aligned with evidence-based guidelines. Scores from 8 to 11 (50–79%) suggest a moderate profile with specific areas for improvement that our targeted programming can address. Scores below 8 (under 50%) indicate multiple wellness domains requiring attention, and we recommend scheduling a free priority consultation with our community health team for personalized support and resource navigation.
